ssessee has to submit the following documents to  reset the password.
FOR INDIVIDUAL:
1. Copy of the PAN card of the Company/Trust/Firm/HUF etc.
2. Address proof
 (Must be same as per Pan or last IT return submitted)

FOR OTHER ASSESSEE
1. Copy of the PAN card of the Company/Trust/Firm/HUF etc.
2. Copy of Proof of the date of incorporation.
3. Copy of the PAN card of the principal contact.
4. Copy of the one more identity proof issued by the Government Agency like Passport/Driving Lincence/Voter ID card/Aadhar card etc.
5. Authorisation letter issued using the in letter head of the Company/Trust/Firm/HUF etc.(having the address telephone etc)

After verifying the credentials, password will be sent to the new e-mail id.
 
It may take 15-20 days for the process

Reference: As per telephonic talk over mobile, you told to email the suggestions so that it can be consider for implementation

1) I have google groups in which more than 1 lakh CAs are members. Thousands of return and TAR is pending as we unable to reset password with new DSC for assessee other than individual.

2) We are unable to file IT return & TAR for all assessees other than individual whose password has been forgotten as we are unable to change password . We are unable to change it with even new DSC as PAN of person whose dsc we are using i.e. say trustee in case of trust and the assessee i.e.trust is different. Hence, Message is showing PAN mismatch.

 ) Kindly modify system to allow trustee / partners etc to use his dsc to change password so that problem can be resolved.
